Can anyone speak to the fit of their custom suits and the mobility of the shoulder area?

40" chest, 30" waist...athletic...only 5'6" so the Dainese I wanted probably would be too tall.... But the Dainese uses alot more Keprotec in the shoulder girdle and looks like it'd move much better.... Also don't want a lumpy "American" fit....

I had a two piece custom suit made for me last winter in time for spring. It has been wonderful!
Fit was excellent; but I must have had my measurements taken half a dozen times. What I can tell you is that you need to take your measurements with whatever back protector and chest pad, etc. you wear when riding. These suits fit close to the body and if I had not worn my Forcefield sub4 back protector and chest pad when having my measurements taken they never would have fit under the finished product.
I rode quite a bit in 90-to-100 degree heat here in the US south and the leathers were extremely comfortable when the wheels are rolling. (I had the fully ventilated option with air intake vents on the shoulders.)
I also toured in the leathers over a week down to Nashville and then over to Louisiana to watch the superbike races at Barber and during that time the leathers performed extremely well. While the leathers do fit close to the body, they do allow you to wear long underwear and a wind suit under them, so they are not super tight, just close fitting. I highly recommend them and would recommend the two-piece without the hump unless you are just racing in them. I would buy them again; I don't think I will ever buy off the rack again.
 
I don't have any experience with Pilot leathers, but you may want to look up RS Taichi leathers. I have the same chest and waist size as you (just a little taller) and run into the problem of extra leather around my waist and upper thigh. Taichi leathers can be bought off-the-rack in about 100 different sizes (however I'd guess there would be a wait time for some of the more rare sizes). They have a Japanese cut that allows for wide shoulders/chest and narrow waist. I got measured by a Taichi rep and found the japanese size to fit me. I started noticing them a lot more over the past year, so I asked around about Taichi and haven't found anyone that disliked them. I was also told that they crash good (pretty subjective, but still good news). The only other leathers I found that fit me decent were Spidi's european leathers, but that was a long time ago and I don't remember which line they were. I don't have Taichi leathers yet, but they will be my next set. They also run a deal sometimes to get a free pair of gloves with the suit purchase.
