Anyone have any additional info on this most interesting mishap?

Found these on a UK board. V4. Rear axle snapped at the flange. Hard to say. Doesn't look like the bike was kept in very clean condition.

I saw this a month or so ago... I believe they were trying to prove the axle nuts were over tightened and cracked the axle. It had only been out of the shop a day or so if i recall correctly
 
Lol, result of setting torque of your rear wheel nut with 1/2 inch pneumatic impact gun! Work on and torquing critical areas like this I only trust one man and that's ME!
 
Not wall impact otherwise the bike would be in a hundred bits, Id say over torqued and a relatively low speed straight line parting of ways; perhaps a bump? As for cleanliness, bike looks pretty normal for daily use.
